RN Case Manager – Nights & Weekends (Friday‑Sunday)

About the role

The RN Case Manager provides after‑hours, field‑based nursing care to patients, families and community partners during Friday‑Sunday night shifts. Working as part of the Chapters Health Team, you will deliver compassionate support, coordinate services and ensure seamless admission processes for our home‑health and hospice programs.

Key responsibilities

• Conduct on‑site visits to assess patient needs, provide clinical evaluation and education.
• Facilitate admissions for all company services, explaining options and addressing concerns.
• Maintain positive relationships with patients, families, referral sources and community partners.
• Document all interactions in the electronic medical record and ensure accurate knowledge transfer.
• Respond to urgent situations, make critical decisions and manage emotional stress professionally.
• Travel to patient locations using personal reliable transportation and a valid driver’s license.

Required profile

• Current RN license in the state of employment.
• Minimum one year of nursing experience; hospice or hospital background preferred.
• Active BLS certification (American Heart Association or Red Cross).
• Ability to work night and weekend shifts on a rotating schedule.
• Strong communication skills in English; Spanish bilingualism is a plus.
• Physical ability to lift patients/equipment and perform sedentary work with interruptions.

Required skills

• Proficiency with an EMR/EHR system.
• Competence in Microsoft Office applications.
• Computer operations, data entry and online research navigation.
• Excellent time‑management, decision‑making and problem‑solving abilities.
