General Practitioner – Part‑time, Chronic Care

About the role

ThriveCare is seeking a part‑time General Practitioner to join its multidisciplinary team in Dar es Salaam. You will deliver comprehensive primary care to patients with chronic and complex conditions, ensuring continuity of care between hospital stays and home management.

Key responsibilities

• Conduct patient consultations, assess health status, and formulate diagnoses.
• Order, perform, and interpret diagnostic tests.
• Prescribe appropriate therapies and develop individualized care plans.
• Coordinate care with nursing staff, therapists, and other specialists.
• Monitor patient progress, adjust treatment plans, and document clinical notes.
• Educate patients and families on disease management and preventive measures.
• Participate in quality‑improvement projects and internal training sessions.

Required profile

• Medical degree with full registration to practice in Tanzania.
• Proven experience as a General Practitioner, preferably in chronic or long‑term care settings.
• Strong clinical judgment and ability to work collaboratively with a diverse care team.
• Commitment to patient‑centered, ethical practice and ongoing professional development.

Required skills

• Competence in primary care assessment, diagnosis, and treatment.
• Ability to interpret laboratory and imaging results.
• Effective communication with patients, families, and multidisciplinary teams.
