Director of Sustainability and Transition

About the role

The Director, Sustainability and Transition provides strategic and technical leadership for designing, implementing, and refining the sustainability and transition approach of the AFGHS‑Jhpiego project across Mainland Tanzania and Zanzibar. Working closely with senior project leadership and government partners, the role ensures that sustainability and government ownership are embedded throughout project planning and execution.

Key responsibilities

• Lead the development, implementation, monitoring and continuous improvement of the project’s sustainability and transition strategy and national/sub‑national transition plans.
• Conduct systematic assessments of sustainability priorities across governance, workforce, financing, service delivery, data, accountability and community ownership.
• Develop and maintain clear transition pathways, identifying functions to be institutionalized, responsible government structures, required capacities, timelines, risks and readiness criteria.
• Advise senior project leadership on transition readiness, sustainability risks and institutional constraints, ensuring continuity and quality of lifesaving MNCH and malaria services.
• Provide senior technical leadership in either MNCH or malaria, ensuring interventions are evidence‑based, aligned with national policies and designed for sustainable scale.

Required profile

• Extensive senior‑level experience in health program management, preferably in MNCH or malaria.
• Demonstrated ability to work with government counterparts to assess institutional readiness and to lead transition and sustainability initiatives.
• Strong understanding of health‑system integration, data quality, and service delivery in low‑resource settings.

Required skills

• Strategic planning and technical leadership in health program sustainability.
• Experience with government engagement, policy alignment and institutionalization of health services.
• Knowledge of MNCH or malaria program implementation and monitoring.
